Please be aware that the insurance coverage verification processed by Revive Acupuncture is not a guarantee of coverage. Misquotes by the insurance carrier are common. It is absolutely necessary for you to confirm the details quoted by Revive Acupuncture with your insurance carrier.

If there is a misquote from your insurance and your acupuncture coverage is less than Revive Acupuncture specified, you will be responsible for paying out of pocket for any insurance denials. It is the responsibility of you alone to understand the specifics of your insurance policy. 

We provide the following verification service as a courtesy for our patients. However, this does not mean that  Revive Acupuncture is responsible for unexpected insurance denials or misquotes in the coverage.
